Title: SERVICE USERS CONSENT (FORM)
1.0 I accept that the Service holds personal data about me and I hereby consent to the processing of my data by the Service or any associated organisation of my personal data for any purpose related to the provision of agreed services.

2.0 I also explicitly consent to the Service or any associated organisation processing any sensitive personal data relating to me, for example medical records, medical reports, particular health needs, details of my religion (as they may apply to the care and services provided) and equal opportunities monitoring data, as necessary for the delivery of appropriate care.

3.0 Finally, I consent to the Service providing my personal data to a third party where this is necessary for the provision of agreed care services, for example to the NHS, my GP or government department.
